Chapel House Admiral Nurse Referral Form

Our Admiral Nurse service is to provide support to those using the services of Chapel House Care Ltd and the local community would not otherwise qualify for an Admiral Nurse, priority is given to Service Users families. We offer regular Cuppa Time drop-in clinics at GIFT Café. Cuppa Time is offered Weekly at GIFT Cafe, Chapel House Lane, Puddington, Cheshire, CH64 5SW Wednesdays 12-2pm.
